A teenager died after his brain was starved of oxygen during surgery at Antrim Hospital because a breathing tube was trapped, an inquest has heard.
Miki McCallum, 13, of Warren Crescent in Portstewart, died at the hospital in January 2005 after an operation to remove his appendix.
The court heard during the operation a breathing tube was trapped between the operating table and a transfer trolley.
Nurses giving evidence said practices had since been changed to prevent this.
They told the court that at the time of Miki's operation, the trolley would be docked adjacent to the operating table while the patient was still connected to anasthesia tubing.
They accepted this was against written manufacturers instructions.
The nurses said the patient's bed was now wheeled into theatre after surgery had taken place.
The inquest continues.
